What are the specific requirements and considerations that international students need to keep in mind when applying to medical schools in the USA and Are there any combined MD and BS programs in the USA that accept international students and offer an accelerated pathway to earning both degrees?

There might be exceptions, but I do not know of any BS/MD program in the US that accepts international students.
If you're planning to attend med school in the US, you cannot apply without getting a bachelor's degree first. You can pick any major for your undergraduate degree, but you'll have to complete med school prerequisites before applying to med school. You'll also need to take the MCAT before applying to med school.
